At Rye, Winchelsea and District Memorial Hospital we offer outpatients, rehabilitation and intermediate services. At Uckfield Community Hospital we provide day surgery and outpatient care.
In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.
Job overview
This is an exciting post in which you will work as part of the Community Heart Failure Team. You will support patients who have a diagnosis of Heart Failure carrying out domiciliary visits and also seeing patients in a clinical setting.
You will need to have experience in carrying out clinical observations, venepuncture and assessment of heart failure symptoms such as oedema. You will support the team in enabling patients to monitor their symptoms.
The post holder will support the Heart Failure Team in delivering an efficient and high quality nursing service using evidence based practice to a wide range of patients within the locality.
It is essential that you have National Vocational Qualifications (NVQ) Level 3 in Health and Social Care or an acceptable alternative Level 3 Qualification, excellent team working and communication skills and the desire to share knowledge and work within an inter-professional framework.
You must have a full driving license as this post involves travelling to patients’ homes and community clinics

Main duties of the job
To support patients who have a diagnosis of Heart Failure carrying out domiciliary visits and also seeing patients in a clinical setting.
To work as part of the Community Heart Failure Service under the direction of the Specialist Heart Failure Nurses. to participates in the delivery of patient care, by ongoing assessment, monitoring and education to optimise the management of people with diagnosed heart failure to improving patients’ quality of life. The post holder, will visit patients in their home, arrange bloods, record blood pressure, pulse, ECG and also recording weight and fluid intake records as directed or requested. to will carry out phlebotomy when required and requested. S/he will carry out administration tasks necessary for the efficiency of the service and support education programmes and rehabilitation groups.

- Proven current experience working in a Health or Social Care Environment

Skills
Essential criteria
- Ability to competently carry out assigned clinical tasks related to the speciality, after training
- Knowledge of medical terminology
- Ability to use physiological recording equipment and accurately record findings and report on them where appropriate

- Knowledge of NICE guidelines August 2010 Chronic Heart Failure
Qualifications
Essential criteria
- Satisfactory General Education including Maths and English
- National Vocational Qualifications (NVQ) Level 3 in Health and Social Care OR an acceptable, alternative Level 3 Qualification
- Demonstrable ability to acquire skills appropriate to provide Scope of Practice that is commensurate to clinical area
Desirable criteria
- Phlebotomy competency certificate
- ECG recording certificate
- Healthcare Assistant Training Course
