Job overview

An exciting opportunity has arisen within the York and Selby Care Home and Dementia team for 37.5 hours per week.

You will need to be enthusiastic and highly motivated and have experience in the completion of assessments and delivery of care for patients experiencing mental health difficulties in a private care providers within the community. The Care Home and Dementia team works collaboratively with the Community Mental health teams to provide assessment and treatment for people with dementia over 7 days.

This post is open to suitably experienced Occupational Therapists, Nurses and Social Workers.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

Main duties of the job

The post holder will be responsible for the management of a caseload of patients and work effectively within a multidisciplinary team. They will also be required to provide clinical leadership and supervision to junior members of the team. There will also be an expectation that the successful candidate will support students and learners within this clinical setting.

The post-holder will be required to travel across the York and Selby area and therefore applicants must be capable of independent travel.

Qualifications

Essential criteria
  • Current professional registration with an approved professional body.
  • Evidence of Continuing Professional Development.
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education.
  • Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
Desirable criteria
  • Leadership or management qualification
  • Post-graduate qualification in a relevant area
  • For registered nurses: Non-medical prescriber

Experience

Essential criteria
  • Significant experience working with people with mental ill health in a community environment
  • Working collaboratively with service users and their families/carers
  • Working in a multi-disciplinary team
Desirable criteria
  • Leadership or management experience

Knowledge

Essential criteria
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
  • Care Programme Approach and its application in practice
  • Detailed understanding of Safeguarding and its application in practice
  • Clinical Risk Assessment and Management and its application in practice
