With a budget of around £365m, three hospitals and 5,500 staff, North Tees and Hartlepool NHS Foundation Trust provides hospital-based services to the population of Hartlepool, Stockton on Tees and parts of East Durham and Sedgefield as well as community, therapeutic and screening to a wider population across Durham and Teesside.

Job overview

Community Matron band 6/7 dependent on experience.

Post is for highly motivated & dynamic Community Matron or aspiring Community Matron to complete & complement our existing team. Working autonomously as an Advanced Practitioner, the post holder will have continuing responsibility for proactively monitoring & managing patients, recognising early symptoms of disease exacerbation, acute illness and injuries based on the understanding of long-term conditions, disease processes, current evidence & practice standards. The service is delivered to a population of patients within community settings primarily residing in care homes.  The key aim of the service is prevention of inappropriate hospital admission or readmission following discharge from acute care.  In achieving these outcomes there will be a reliance on comprehensive physical assessment & treatment-planning along-side consideration of Emergency Health Care Plans and DNACPR as supportive tools.

The post involves liaising with a variety of health & social care providers & other agencies to ensure the provision of high quality care management to this group of patients and their carers.  In addition post-holders will work closely with the Care Group nurse-led unit a&community hub located on the University Hospital of Hartlepool site.

Applicants with limited prescribing & physical assessment experience should expect to commence on Agenda for Change band 6, with a clear development plan put in place to attain band 7.

Post involves liaising with health/social care providers & other agencies to ensure provision of high quality care management to this group of patients/carers. Post-holders will also forge strong links with the nurse-led unit and community hub which is located on the University Hospital of Hartlepool site delivering pathway based interventions.

The Community Matron service operates over 7 days a week, covering the hours of 08.00-20.00hrs across the Hartlepool and Stockton localities.

The Community Matron service is a team of highly skilled advanced nurse practitioners working within Care Group 1 of the Trust (Healthy Lives).

The matron deliver reactive & proactive interventions to people residing in care homes across the Hartlepool & Stockton CCG area.

The key focus of this team is to ensure that people are cared for in their own home environments & that hospital admission is prevented where possible.
