Trosolwg o'r swydd

As part of community transformation an exciting role has been developed between TEWV & Tees Valley Primary Care networks (PCN’S).

We are looking for 2 enthusiastic Community Associate Practitioners to work within the Tees Valley- one post holder will cover the Middlesbrough Locality and one will cover the Hartlepool Locality.

The role is to ensure that physical health needs of SMI patients are met.  The practitioner will be expected to link in with individual GP practices to identify patients outstanding for physical health checks.

An assertive outreach approach will be adopted to engage patients on the SMI register to have their physical health investigations carried out.   A kit will be provided to ensure that physical investigations can be carried out, at times,  outside of a clinical environment.  Full training on the kits will be provided.

Time will be shared between TEWV & the PCN meaning the new role will be carried out alongside duties of an Associate Practitioner within TEWV.
