We are currently seeking a dynamic, enthusiastic, Community Staff Nurse to come and join our vibrant and innovative North Bournemouth Frailty Team (NBAT) to support the health of our community in Bournemouth North PCN.

This is a part-time permanent post of 22.30 hours per week. The post holder may be required to work flexibly to meet the needs of the service.

As a community staff nurse you will be providing nursing care to patient in their own home. We provide a wide and varied level of care to patient in their home. This means we have highly skilled workforce; successful candidates will be supported with upskilling and training. 

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Main duties of the job

As a community staff nurse you will provide a high-quality person-centred approach to care delivery which always considers people’s safety, privacy, and dignity. 

As a member of the multi-disciplinary team you will be participating in yearly reviews for housebound patients with long-term chronic conditions QOF.

Undertake assessment of patients / service users care needs and develop programmes of care as part of the wider multi-disciplinary care team and signposting to other services/voluntary sector as appropriate.

Assess individuals needs using tools such as frailty, 6CIT and pressure ulcer risk assessments and other DHUFT tools.

Clinical skill to include bloods, ECG, participate in vaccination programme for older patients and any other clinical skills within your limitations.

To provide advice to patients/ service users and carers regarding their care.

To plan, implement and review health improvement programmes in a range of settings.

Promote difficult conversations in terms of future care needs, such as Advanced care planning.

To ensure timely data entry of visit onto system one.

Provide supervision, mentorship, and clinical support to students and junior staff in the team.
