Job overview

An exciting opportunity has arisen for a Band 6 Community Nurse Specialist to join the Out of Hours Community Nursing Service.  The post is to cover 33 hours for the evening hours of 19:00 to 07:00. 

The Out of Hours team provides responsive, planned and unplanned support to patients requiring urgent nursing within their own homes/residential care settings. 

Staff are expected to work autonomously and must be prepared to travel and organise time accordingly.

We have technology and lone working procedures in place to support staff during visits.

You must also be able to care for people who live in the community, who are at risk of deteriorating health resulting in a reduced quality of life, with an emphasis on avoiding hospital admission or unnecessarily long lengths of stay. We are looking for a dynamic, enthusiastic and motivated individual to join our Out of Hours Community Nursing Team.  

You should have excellent communication and interpersonal skills and the ability to organise and prioritise your workload without direct supervision. You should be committed to delivering a high quality service through an integrated and holistic approach to patient care.

Main duties of the job

  • To provide holistic nursing assessment, high quality care, support, advice and information to patients and carers with the aim of maintaining patients in their own homes, enabling them to enjoy maximum independence and optimum quality of life
  • To ensure the effective co-ordination and provision of the Nursing Service in the absence of the ISU Nurse Lead
  • To actively participate in the planning and discharge of complex patients from secondary to primary care
  • To provide nursing interventions, assessment, liaison, referral, co-ordination, delegation of care, or placement in order to prevent hospital admissions
  • To provide specialist clinical advice to the multi-disciplinary team
  • To manage a team of Community Nurses and SNR’s in line with the trust policy

Detailed job description and main responsibilities

  • As the first clinician to screen and assess a patient, the post holder will be expected to plan and implement treatment to ensure safe and effective delivery of care. In consultation with the Health and Social Care Team, this may require placing the patient in a Place of Safety bed or arranging immediate domiciliary care
  • To communicate formally and informally with the ISU CN Team in order to ensure best practice and consistency with the community and intermediate care service in Torbay & South Devon NHS foundation Trust
  • To be responsible with the ISU leads for the monitoring of standards in order to ensure that the service consistently meets the needs of the population and adheres to NMC and Trust requirements
  • To promote health education and well-being when visiting patients, relatives and carers
  • To participate in further training, maintaining a commitment to professional development, using and participating in research and audit to continually improve and extend the knowledge of the team and patient care outcomes
  • To utilise teaching, assessment and mentoring skills with students and team members and to provide an effective learning environment
  • To communicate effectively with hospital, practice and crisis response teams, Devon Doctors, community matrons and Intermediate Care services and the wider community
