Are you a Advanced Nurse Practitioner? Utilise your clinical expertise to make an impact in our Luton Town Centre Surgery.

Working as part of a multi-disciplinary team you will assess and treat patients in the UTC who may preset with complex and/or chronic illnesses and/or injury; in order to determine a clinical diagnosis, treatment and appropriate discharge.

 

Main duties of the job

Using your streaming/triage experience gained within an urgent treatment centre/emergency department or minor injury unit, you will receive patients as they present with complex and/or chronic illnesses and/or injury, that have been triaged and referred by the patients own GP or 111. Autonomously undertaking clinical assessments, to determine a clinical diagnosis, treatment and appropriate discharge.

Detailed job description and main responsibilities

  • To autonomously undertake clinical assessments, diagnose, treat and discharge patients with    
  • a wide range of minor illnesses and injuries, including management of complex cases
  • To work as a key member of the multidisciplinary team to provide effective face-to face assessment within the UTC to ensure that patients are streamed to the correct clinician with the correct clinical priority for their condition
  • To stream and prioritise self-presenting/referred patients to Luton UTC at the front door.
  • To support access for patients within the UTC service opening hours and facilitate a clear patient pathway
  • To offer advice and health education as appropriate
  • Enable patients to carry out self-care as appropriate, through effective communication of health care advice.
  • Recognize the acutely ill patient and the need for urgent/emergency treatment acting appropriately with other clinical support as required
  • Identify any safeguarding concerns and make appropriate referrals following safeguarding processes
  • To be committed to working in a changing environment, responding positively to new demands and changes and to be willing to be flexible to the changing needs of the service
  • To promote and facilitate audit and research-based practice as required
  • To direct patients appropriately to the right professional within appropriate time frame
  • To assess and priorities presenting patients in terms of acuity whilst maintaining patient safety at all times.
  • To undertake ECGs, chaperoning and other tasks as requested by colleagues, this is not an exhaustive list.
  • To support clinicians and other members of the team to ensure timely care to patients.

  • Participate and work to the policies and quality standards set by HUC and the Urgent Treatment Centre.
  • Ensure that all the requirements of revalidation are met

 Communication

  •  To work closely with all members of Luton UTC team, Town Centre Surgery and the out of hours team within the clinical hub as appropriate. To communicate professionally and effectively with patients /relatives / carers /colleagues
  • To keep accurate and contemporaneous records of all health enquiries, including assessment, treatment, and advice regarding patient care. The documentation must clearly reflect the assessment and findings in the consultation in line with current practice and in accordance with The Code as stipulated by the Nursing and Midwifery Council
  • To attend and participate in UTC meetings as required

 

Person specification

Essesntial

Essential criteria
  • NMC Registered adult/ dual trainned Nurse
  • Triage/streaming experience
  • Evidence of working with a variety of patients requiring triaging assessment and treatment (minimum of 1 year experience)
  • MSc in Advanced Clinical Practice or equivalent credentialing
