Job overview

We currently have an opportunity for a dynamic, highly motivated, innovative and forward-thinking Advanced Clinical Practitioner (ACP) to join our newly developed Acute Home Visiting Service.

The post will offer the opportunity to also cover the Community Support Beds to further increase the variety of the role.

You will be required to work autonomously with the flexibility to manage a wide range of patient groups.

The ACP is an integral and significant member of the multi-disciplinary Team (MDT).

You will be part of the DCHS team delivering care in Swadlincote whilst also working closely with Swadlincote Primary Care Network.

This is an exciting time to join our workforce based in South Derbyshire.  DCHS are supporting the Swadlincote PCN to deliver Team Up!

Team Up is a Derbyshire wide service teaming up community services from the Healthcare, Social care, Charitable and Voluntary sectors to provide a comprehensive package of care for housebound patients. 

As an ACP,  you will  work directly with and support the GPs and be a major contributor to the success of this new service.

Alongside this, is the opportunity to work in a lead clinical role within Oaklands Village in the centre of Swadlincote, a modern, purpose-built facility, offering Pathway 2a Community Support Beds.

Main duties of the job

The role will provide clinical input to the residents at Oaklands Village, Swadlincote, including managing the MDT.

It will also support the newly formed Home Visiting Service (HVS), working with the GP’s to triage referrals, undertake home visits and provide appropriate care for housebound patients.

The role requires a high degree of autonomy and the successful candidate will have excellent interpersonal and communication skills.

The candidate will be a confident and adaptable clinician who can work at an advanced clinical level using independent prescribing & clinical guidance documents.

The successful candidate will contribute to the development of relevant policies and protocols whilst delivering service improvement for the service.

This role will involve travelling to housebound patients across the South Derbyshire area as and when required.

There will be a requirement to work evenings and weekends.

A Band 8a position will be offered to candidates who have already completed an MSc in Advanced Practice (or equivalent) and who can demonstrate contemporary experience working at an advanced level within a relevant clinical environment.

Eduaction, Qualifications and Training

Essential criteria
  • Professional registration with NMC or HCPC
  • Evidence of post-registration study at undergraduate level
  • MSC in Advanced Clinical Practice or equivalent

Experience and Knowledge

  • Experience of a leadership role
  • Contemporary clinical experience of older people with complex needs
