The Community Diabetes Nurses are a small friendly team of specialist nurses. We support management of patients with Diabetes across North Derbyshire.
This includes a mixture of face to face nurse led clinics, remote consultations and education. This is a very varied role working with adults of different ages, to maximise their quality of life, living with Diabetes.
This role is suitable for development from Band 6 to band 7 once competencies are met or as a Band 7 role depending on experience and skillset.
Prif ddyletswyddau'r swydd
Provide a comprehensive service to adult patients with diabetes referred to the service
Work in collaboration with GP’s and consultants within North Derbyshire area
Work with other members of the DSN team to support the delivery of community based education to individuals and groups within North Derbyshire to promote high quality support for adults with diabetes
Work as part of the team to deliver training to a range of professionals to support with their care of people with diabetes
Be responsible for maintaining competency to practice through maintenance of professional registration, continuous professional development activities and maintain a portfolio of evidence of learning beyond registration.

- To perform comprehensive nursing and diabetes assessment with/for patients.
- To be responsible for formulating a diagnosis and developing, planning and delivering an effective individualised treatment programme in partnership with patients.
- To continually evaluate patient progress, reassess and amended treatment as appropriate as part of an individualised treatment programme in partnership with patients
- To be responsible for assessment and management of clinical risk within own case load
- To be professionally and legally accountable for all aspects of professional and clinical work
- To communicate in timely verbal and written reports and discharge summaries to other agencies as appropriate
- To communicate effectively with patients, to gain informed consent to treatment and agree expected outcomes.
- Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national/local protocols – non-medical prescriber (for band 7).
- Motivate patients and where appropriate carers to enable agreed treatment programmes are adhered to

Qualifications

- Registered General Nurse or Registered Nurse for learning disability or RMN (or equivalent)
- Evidence of post registration training in Diabetes

- Non-Medical Prescriber or willingness to undertake relevant training
- Teaching/mentor qualification C&G730 ENB 998 or equivalent
- Diabetes qualification
Experience

- Good understanding of the national diabetes agenda, prevention and treatment
- Evidence of working clinically at a senior level
