Specialist Nurse -Leaving Carers/UAS

An exciting opportunity has arisen to join our established Vulnerable Children and Young People’s Team as a Specialist Band 6 Nurse on a 12 month fixed term contract/secondment on a part time or full-time basis.  Based at Clay Cross Clinic, this post will work closely from Care Experienced/UASC Hubs in both North and South Derbyshire in addition to Clay Cross Clinic and other community venues.  

Looking for a Specialist Registered Children's Nurse/Learning Disability and RN Adult (also working with young people post 18) for UASC and Care Leavers to join our amazing team on the journey to improve health outcomes for vulnerable children and young people to ensure that their voice is heard. To develop this new role  working closely with the Children in Care Team to improve understanding and management of  health needs of care experienced young people and unaccompanied asylum-seeking children. 

Working collaborative with Derbyshire County Council by working closely with the Leaving Care and UASC Teams to improve access to health services, identify and review outstanding health needs and complete statutory health assessments for this vulnerable group of young people.   The role will require the development of multi-disciplinary relationships and approaches by listening to young people to develop the role of a Specialist Nurse that understands and meets their needs whilst adhering to statutory responsibilities.

To be responsible for the assessment, review, and promotion of positive health for care experienced young people and UASC across Derbyshire. You will act as a direct link between health and social care to improve outcomes for referred care leavers and UASC, who may be faced with challenges when accessing services. This role will also provide support and direct contact post 18 inline with Care Leavers and UASC Teams.

To undertake Health Assessments for Care Leavers/Care Experienced young people and UASC as requested by social care and children in care team.   Undertake complex health assessments for hard-to-reach children and young people.  To engage with, and listen to young people, across a range of issues that affect their health and personal development. 

To develop and implement health promotion / public health strategies for individual Care Leavers and UASC in foster care, residential settings, and semi supported living in line with government recommendations and local
targets.

Qualifications and Training

  • Registered Children's Nurse/Learning Disability OR as a Specialist Community Public Health Nurse

  • BA Hons Community Nursing (or equivalent)
  • DIP Level 2 Modules in any of Family Planning, Asthma, Epilepsy, Counselling, Health Promotion, Continence Promotion.

Experience

  • Multi-agency working in School Nursing or Public Health Nursing with Primary Care Health Providers.
  • Experience of working in the Community.
  • Experience of lone working.
  • Experience of working with Vulnerable Children and Young People.
  • Experience of attending and participating in Safeguarding meetings.
  • Trauma Informed approaches.
  • Writing detailed reports.
  • Working towards Statutory timeframes.
