  •  37.5 hours - RPC West -Community Practice Nurse/Nursing Associate

We have a team of  over 180 people dedicated to our patients, working across 3 sectors covering  Chesterfield, Clay Cross and Heanor.  Our Practice Nurses are essential to ensuring that we always provide an outstanding level of nursing care to our patients.

Main duties of the job

The  Community Practice Nurse will undertake a diverse range of nursing assessments and provide the appropriate care / treatment in conjunction with GPs, other Practice Nurses or independently as appropriate for the patient needs, according to practice policy and protocols. The Practice Nurse will strive to improve standards of care and be involved in identifying practice population needs.

The successful candidate will work at Royal Primary Care, a multi-site organisation, working within the team to enhance nursing services within the community. Long Term Chronic Disease management will form a large part of the role. Knowledge of the GMS contract, e.g. The Quality and Outcomes Framework, will be valuable to the post.

Qualifications

Essential criteria
  • Registered General Nurse
  • Registered qualification level, diploma or above
  • Evidence of a commitment to continued professional development
Desirable criteria
  • Further accredited qualification in any chronic disease
  • Mentorship qualification and/or sign off mentor

Experience

Essential criteria
  • Experience of working in accordance with the NMC Code of Professional Conduct
  • Experience of dealing with the public in a respectful and professional manner
  • Experience of a multi-skilled approach to Chronic Disease Management
  • Understanding the need for confidentiality at all times
Desirable criteria
  • Practice qualification or experience of working in primary care
  • Evidence of working in a complex organisation, demonstrates sound knowledge or awareness of the role of a nurse in primary care
  • Experience of TPP SystmOne clinical computer system or equivalent
  • Evidence of involvement in research, training and development
