What does the Community Response Team do?

They step in to support people who are either discharged from acute hospitals or experience an acute change in their function in their own home, and need short term/interim enablement support at their homes.  The teams are committed to supporting people at home and work over a seven day rota.

What are the hours?

The shift pattern is Early-0800-1400 and Late 1600-2200 over a seven day rota and we have full time hours available (36 hours) along with part time options minimum of 24 hours per week.

Where would I be working?

Working in this team means you will need be able to meet the travel requirements of going to people’s homes, so you must have access to a vehicle which can be used for work purposes . DCHS has a mileage policy and car lease scheme available to support our staff.

The successful candidate is expected to work and document to a high standard and work in a way that reflects the DCHS values.

To maintain standards, ensuring quality of patient care through the development of a range of knowledge and competencies to support clinicians.

To assist all clinicians in delivering agreed programmes of treatment and care where appropriate and work as an effective member of the wider multi-disciplinary team.

To carry out individual care without direct supervision in the community.

To provide support to patients to optimise their health and independence.

The Purpose and Activities of DCHS?

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
