Job overview
An exciting opportunity has arisen for a Care Navigator to join the East Integrated Care Community (ICC) Case Management Team.
East ICC covers Arnside, Milnthorpe, Sedbergh, Kirkby Lonsdale and Bentham areas. This post will be required to cover the whole of the East ICC patch (and Carnforth ICC area on occasions).
As the Care Navigator you will meet with people in their homes to understand their health and social needs. You will coordinate their care, offer advice and organise additional support. As such your role will involve working closely with other teams from both statutory and voluntary organisations.
The team regularly attends external health promotion events and you would be expected to support these.
Main duties of the job
There is a requirement for the post holder to have the Assistant Practitioner role (or a willingness to commence training) along with excellent communication skills. Computer literacy is also essential, alongside good organisational and time management skills.
The Care Navigator will be based at Tarnwater Offices, A6, Yealand Conyers, LA5 9RJ. As a team, we cover a broad geographical area and an ability to travel across the ICC footprint is expected in order to be able to meet the needs of the local population.
Working for our organisation
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
