Brampton and Longtown District Nursing Team are an emerging and unique team that put people at the heart of what we do.  The ICC empowers and engages its workforce to improve care and wellbeing, by delivering better health and better care in partnership. The ICC is a well-established team with long-standing working relationships with many different partners and stakeholders. Effective collaboration with Primary Care, Social Care and multiple charities and third sector providers enables us to deliver a wide-range of services to the population we serve, covering rural and urban  locations and have good team spirit and resilience. We work as part of the larger Carlisle and District ICC.

We are looking to appoint a motivated and enthusiastic Senior Community Staff Nurse to work as part of our dedicated Community Nursing team and the wider ICC. The role involves the delivery of holistic, patient-centred care in the community as well as complex caseload management. The successful applicant will be expected to liaise and work with the primary health care team and other Health and Social Care professionals in care provision. This is an exciting time of change and development within the ICC which provides an opportunity for professional development.

You must be able to demonstrate the ability to work within a changing environment and have good communication skills. The successful applicant must be able to demonstrate the ability to work on your own, and also as part of a team. You will be supported by the District Nursing Team Leads and Band 6 District/Community Nurses to deliver nursing care covering weekdays, weekends, bank holidays and evenings.

This role requires you to demonstrate the skills and knowledge to support the provision of high quality care, to support patients with complex needs within the community setting, and to support the Team Lead to provide clinical leadership and operational management of the community nurse team along with management of the clinical caseload for the team. Community experience is therefore desirable.

This is an excellent opportunity for dedicated and committed registered nurses who are looking to develop their clinical skills and expertise in caring for patients, working closely with health and social care colleagues to prevent hospital admission, facilitate hospital discharge and promote independence. This post will be a development role.
