An exciting opportunity has arisen for a full time enthusiastic, vibrant, Senior Staff Nurse, to work in our community  branch of the Mental Health Rehabilitation Service.

The Community Mental Health Rehabilitation team provides a service across the whole county of Cornwall supporting people recovering from complex and enduring mental health issues. 

The Community Rehabilitation Mental Health team works collaboratively as part of a wider structure within Cornwall Foundation Trust, this includes carers, families, other specialists e.g., Early Intervention Team, we also assist with the discharges of patient from the acute wards and work alongside housing providers, education, pharmacy,

We take a person-centred approach in developing individualised dialogue plus care plans. In addition, the post holder will have a key role supporting discharges from the acute wards, supervising the band 4 social inclusion team and lead HCAs as well as participating in the continued review and development of the rehabilitation service. Regular individual support and supervision is provided. This is a full-time post following a shift pattern

  • To deliver a high quality mental health service to those individuals with severe and/or enduring
    mental health problems.
  • Being responsible for the assessment, planning, implementation and evaluation of persons with
    mental health needs. Maintaining clinical documentation, ensuring robust care plans and risk
    assessments are in place and reviewed regularly.
  • To provide clinical advice, education, support and training to junior staff within the team and
    primary healthcare teams, other professionals, relatives and carers on the management of
    mental health problems.
  • Participate in caseload management.
