Job overview
We are looking for a positive and enthusiastic professional with a nursing, social work or an allied health professional qualification who is looking for a new challenge or next step in their career to join the Continuing healthcare (CHC) assessment team.
This important role is central to the delivery of CHC across Cornwall and Isles of Scilly with a primary objective of ensuring fair access to CHC with better outcomes and better experiences for our patients.
Our ideal candidate will be able to work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) case managing clients to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with complex health needs live their best lives. You will be passionate about patient care and improving life opportunities.
Main duties of the job
You will promote good professional practice in CHC by advising and liaising with other health providers, care providers and Adult Social care colleagues. You will be accountable for managing a caseload of Continuing Healthcare assessments, reviews and placements which will include care planning.
To be successful you will be able to demonstrate the ability to remain calm, to handle emotive, complicated, and sensitive issues. You will need to show tact and diplomacy, good workload management and be able to negotiate with confidence.
As part of the county wide team work flexibly to respond to the demands of the service to support the continuing healthcare process. The ability to travel independently to visit patients at various locations across Cornwall is essential.

Qualifications, training and professional membership
Essential criteria
- Registered allied health professional in mental health, physical health or learning disability
- First level registered nurse in mental health or physical health or learning disability
- Qualified social worker
Desirable criteria
- Educated to degree level or having equivalent experience of 3 years clinical leadership/organisational management experience
Experience
Essential criteria
- Clinical or social work background and proven knowledge of either physical, mental or learning disability heath
- Substantial experience post registration
- Experience of working as an autonomous practitioner
- Experience of using time and resources effectively
Desirable criteria
- Experience and understanding of the Continuing Healthcare process

- Excellent interpersonal and communication skills with the ability to communicate complex issues and negotiate at all levels
