Community Staff Nurse (Richmond District Nursing Team)

We are currently seeking an enthusiastic and motivated Band 5 nurse to join our Community Nursing Team as a Staff Nurse within the Hambleton & Richmondshire locality based at The Friary Clinic, Richmond
There is an expectation that you may be required to work in another team within the locality at times to cover service need.
As a community nurse, you will provide high quality, invaluable holistic care for people in their own homes and residential settings which requires a variety of skills including wound care, continence management, palliative care, health promotion, venepuncture and more. Candidates will be supported with an induction period. This is a 7 day service with shift patterns to cover between 0800 - 1800 hours.
The District Nursing Teams are attached to a Primary Care Network (PCN), to provide coordinated care through integrated teams. Through integrated working, a significant change in the way practices interact not only with each other, but the wider health and social care system.
This will enable the traditional barriers to the delivery of health and care to be broken down to enable care to be delivered to a defined patient population, in a different way to meet the needs of that population, focused on prevention and personalisation of care, making best use of resources collectively.
Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.

The Community Staff Nurse is expected to provide clinical care, innovative practice and support to the District Nurse Sister and Clinical Lead to our ever developing teams. This role will provide the opportunity to work closely with GP practices and other partners, in supporting patients with highly complex and often high acuity needs to prevent unnecessary hospital admission.
This role requires an experienced clinician with team working and communication skills, delivering effective evidence based care to a defined demographic population with diverse health care and social needs.
The successful candidate needs to be prepared to adapt to the changing needs of the Trust and the community models of care as they arise.

You will be expected to attend the leadership development programme, and the Quality Improvement Programme that the Trust delivers. This programme aims to:

• Develop your Leadership effectiveness and skills
• Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role.
After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3,5 and 7.

- Diploma or Degree in Adult Nursing

- Evidence of Post Graduate clinical training/ development
Experience
