An opportunity has arisen for a dynamic practitioner who can evidence personal and organisational development to apply for the post of Critical Care Outreach Practitioner. Applicants must have evidence of significant experience in an acute or critical care environment as essential criteria.
The applicant will utilise specialist knowledge and skills to assist in the delivery of the Critical Care Outreach Service to ensure that patients at risk of clinical deterioration or that are deteriorating have their needs prioritised with appropriate assessment, intervention and ongoing management.
Main duties of the job
They will assist with the development and delivery of education to equip healthcare professionals with the knowledge and skills required to detect patients developing a critical illness and to respond appropriately using a physiological track and trigger system.
In liaison with the Nurse Consultant in Critical Care the post holder will support and improve services for patients in ward areas who are at risk of deteriorating and may require admission to level 2 or 3 critical care areas. In addition to this discharge from Critical Care areas will be supported by the Critical Care Outreach team ensuring post ICU patients receive appropriate physical and psychological support in line with national guidance.
The applicant must demonstrate exemplary interpersonal skills and have the ability to effectively communicate with all members of the multi-professional team. Their role will epitomise the promotion and facilitation of good practice to ensure the delivery of high standard quality patient care.

Essential criteria
- Registered Healthcare Qualification
- Degree or Diploma in Healthcare
- Clinical/physical assessment skills course or working towards
Desirable criteria
- Recognised Acute/Critical Care course or working towards
- ALS provider
Knowledge
Essential criteria
- Evidence of specialist knowledge in management of the acutely ill patient
- Evidence of diverse specialist knowledge as the service covers all areas of the hospital
- National strategic policies in Critical Care Outreach Services
Desirable criteria
- Data collection and audit

You must have appropriate UK professional registration.
