Heart Failure Specialist Nurse

Job overview
An exciting opportunity has arisen for a dynamic individual to join our well-established multi-disciplinary team as a specialist nurse.
We are looking for a highly motivated, experienced cardiology nurse with excellent interpersonal skills, complemented by a high level of clinical expertise and qualifications relevant to the post.
Main duties of the job
You must be highly motivated and be an autonomous practitioner ensuring optimisation of medical therapy, extensive education and support to patients and relatives/ carers aiming to prevent unnecessary hospital admissions/ re-admissions and maximise patient’s quality of life.
You will also be involved in data collection for the National Heart Failure Audit. The post requires you to support ward and clinical staff in heart failure patient management plans and discharge planning. Experience working in the area of specialist heart failure care is desirable, with proof of clinical competencies.
You can expect to be part of a dynamic, committed and constantly evolving multidisciplinary heart failure team, offering ongoing support with an initial supernumerary introductory period.

- Formal teaching qualification

- Experience of working collaboratively with multidisciplinary professionals
