The post holder will:

ensure assessments are undertaken in line with the National Framework for Continuing Healthcare and funded-Nursing Care and submitted to the ICB within the service’s key performance indicators.
commission appropriate packages of care for individuals eligible for CHC or other NHS funded care, in line with the Commissioning Policy.
provide case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the National Framework for Continuing Healthcare and NHS-Funded Nursing Care.
manage complaints/disputes.
process urgent fast track referrals and, where appropriate, procure packages of care in a timely manner for individuals who are receiving or require end of life care.

Qualifications

Essential criteria

Educated to degree level or equivalent level of qualification or experience in working at a similar level in specialist area
Nursing qualification or equivalent, for example Occupational Health, Physiotherapist, Social Worker etc with current registration.

Knowledge

Essential criteria

Understanding of Local Authority functions.
Conversant with national and local guidance on NHS continuing health care and NHS Funded Care
Strong understanding of the operational management and clinical role of the NHS
Understanding of the patient experience agenda & its contribution to quality
Knowledge & understanding of the current NHS agenda and of primary care
A good understanding of personalised care, in particular personal health budgets

Desirable criteria

Understanding of the commissioning agenda including its scope, structure, methods of operation, processes and inter-relationships

Every three minutes someone in the UK develops dementia: a progressive and complex condition that can be devastating for the whole family. One in two of us will be affected by dementia in our lifetime – either by caring for someone with the condition, developing it ourselves, or both.

Dementia UK is the specialist dementia nursing charity that is there for the whole family. Our specialist nurses, known as Admiral Nurses, provide free, expert advice, support and understanding to help families care for their loved one. Every day, Admiral Nurses help families up and down the country to have the best life possible, for as long as possible.

Purpose of the job/about the role

Dementia UK has entered into an exciting three-year corporate partnership with a national partner. As a key component of this initiative, we will establish face-to-face Admiral Nurse clinics across the UK. This new team will bring Admiral Nursing to high street locations across England, Scotland, Wales, and Northern Ireland.

As part of the national Clinics Programme Team, you will provide in-person biopsychosocial support and advice, sharing your specialist dementia nursing expertise to help families manage the complexities of the condition. Your focus will be on supporting family carers, people affected by dementia, and professionals who require specialist dementia guidance in areas of the UK where Admiral Nurse services are currently unavailable, bringing a much-needed service directly to local communities. Throughout your work, you will consistently promote Admiral Nursing and a family-centred approach to dementia care.

You will receive full support with your continuous professional development from the Admiral Nurse Academy.

As this unique nursing role offers you the opportunity to travel across the country, and will include overnight stays, flexibility is essential. When not delivering face-to-face clinics, you will provide support on our Helpline, working from home.

Admiral Nurse – Delivered Services

Join our Delivered Services team as an Admiral Nurse and bring your specialist dementia nursing expertise to support family carers, people affected by dementia, and professionals who require specialist advice. You’ll be a key member of our team, providing biopsychosocial support through our National Dementia Helpline and Virtual Clinic appointments.

In addition to your core responsibilities, you’ll have the opportunity to lead and support other clinical projects across the charity, benefit from monthly clinical supervision, and participate in a range of learning opportunities to support your ongoing professional development.

As an Admiral Nurse, you’ll be responsible for providing nursing support and expertise, offering practical guidance to colleagues, the general public, those with dementia, and others in supporting family and relationship-centered approaches to dementia care. You’ll also lead and coordinate shifts as required, providing clinical supervision to others and assist with the development, evaluation, audit of the service, carer and supporters in different settings.

To be successful in this position, you’ll need to be an active Registered Nurse on the NMC register with a post-registration qualification or training in dementia care or a relevant subject. You will have significant experience of practicing as an Admiral Nurse or dementia specialist nurse and previously worked with people who have dementia and their family, carer and supporters in different settings.

Strong communication skills are essential, as is a proven track record of providing high standards of clinical leadership and specialist nursing support. You should be adept at providing clinical advice, clinical supervision, and advocating for family carers.
