Job overview

**This position is open to staff working at the Trust in the ‘ECT Family’ this includes current East Cheshire NHS Trust employees, both NHSP and ECT bank staff, ISS staff and volunteers at the Trust. Unfortunately, we cannot accept applications from applicants not listed within these groups.**

The Macclesfield District Nursing team are looking for a Community Healthcare Assistant who possesses the enthusiasm and positive approach to join a multidisciplinary team with a vision to deliver the right care, at the right time to help improve the lives of our residents.

Our team is based at Waters Green Medical Centre in Macclesfield and supports a population of over 66,000 people. 

This is an exciting opportunity to join our innovative team to drive forward the expansion and modernisation of community nursing services with commitment to deliver the NHS Long Term Plan.

Travel is an essential part of this role. Please demonstrate on your application form how you can meet this requirement.

Hours per week: 12 - split over at least 3 days

Main duties of the job

  • To contribute and support the delivery of a high standard of effective care, as part of a multidisciplinary team, offering face to face support under the direction of a registered professional
  • You will ensure that patients/clients receive safe, individualised and effective care which supports health & wellbeing and promotes self- care where appropriate and possible.
  • To provide individualised plans of care to support patients wishing to cease smoking 
  • To provide care to people in their own homes

Qualifications

Essential criteria
  • Good standard of general education or ability to demonstrate equivalent.
  • Good literacy skills
  • Care Certificate (or commitment to undertake on commencement of the post)
  • NVQ level 3/ City and Guilds – Health/social care related subject (level 3) – or equivalent experience.

Other

Essential criteria
  • Must be able to undertake all aspects of the role whilst working as a lone worker
  • Flexible approach to meet service need including ability to work flexibly across a variety of shift patterns

Skills, Experience and Abilities

Essential criteria
  • Experience of working with people and dealing with members of the public
  • Self- motivated and enthusiasm for the role with the ability to work on own initiative whilst recognising own limitations
  • Demonstrates effective written and verbal communication skills
  • Computer literacy

  • Experience within a caring environment
  • Experience of working within a multi-disciplinary team
  • Previous experience delivering care in a community setting

  • Knowledge, skills or experience in health/social care

  • Knowledge of Microsoft office systems
