Job overview
We are looking for an experienced professional willing to develop with excellent clinical and operational leadership skills who can demonstrate critical thinking with confidence in decision making, enthusiastic, and has initiative and positive approach to teamwork to drive the community nursing service forward.
The candidate must have a strong effective leadership and organisational skills, problem solving and change management skills, with sense of accountability and ability to work under pressure.
Hours per week: 37.5 with a variety of shifts.
Main duties of the job
To be responsible for the oversight and operational management, leadership and coordination of the integrated community team across the designated geography and to be professionally accountable for the 24-hour delivery of patient care by the integrated team.
To lead and support the team to provide a patient centred service based on a strong supervisory leadership that has high visibility in practice.
To hold line management responsibility for both human and financial resources of the Out of Hours Team
To be responsible for implementing and monitoring adherence to agreed policies, protocols and clinical standards, ensuring the effective and efficient delivery of high quality service provision within the bounds of clinical governance.
To ensure that services are safe, caring, effective, responsive, cost effective and well led.
To provide guidance and support in the implementation of clinical standards and quality initiatives in line with the Trust’s clinical risk management framework.
To provide quality monitoring for patients on the caseload supporting the delivery of expert clinical care and providing leadership and support to all team members within sphere of responsibility.

Qualifications
Essential criteria
- Current professional registration with a nationally recognised professional body (NMC)
- Mentorship qualification (or willing to undertake)
- Clinical supervisor (or willing to undertake)
Desirable criteria
- District Nurse SPQ (Specialist Practitioner) qualification
- Palliative care qualification
- V150 nurse prescriber (or willing to undertake)
- Leadership / Management training or qualification at degree level (or willing to undertake)
Knowledge & Skills
Essential criteria
- Evidence of involvement in clinical audit
- Wide understanding of community services and commission intentions
- Knowledge of muti-disciplinary service delivery for adults and older people across health, social and voluntary sectors
- Evidence of innovative practice and evaluation of outcomes
- Knowledge of NMC and local standards for record keeping and confidentiality
- Evidence of commitment to sustained professional development including leadership
- Evidence of clinical expertise and caseload management
Desirable criteria
- Knowledge of Commissioning / Provider services
- Understanding and insight into the implications of the NHS long term plans in relation to local agenda
- Clinical supervisor and / or coaching skills
- Sign off Assessor / Supervisor
- Evidence of change management and service development
Experience

- Significant post registration clinical experience in relevant field
- Experience of managing a team and supervising staff
- Experience in managing End of Life Care at home
