An exciting opportunity has arisen to join our innovative Hospital at Home service and Care Community teams in East Cheshire.
Are you a compassionate and skilled nurse with a passion for making a real difference in people’s lives? We’re looking for a dynamic and proactive Community Response Nurse to join our team and provide urgent, high-quality care to patients in their own homes across the five unique care communities of East Cheshire.

As a Community Response Nurse, you’ll play a critical role in supporting patients requiring urgent care within the community. Responding swiftly to call-outs, you’ll assess patient needs, administer treatment, and provide reassurance and guidance to patients and their families. You’ll collaborate with an interdisciplinary team, including GPs and community health workers, to ensure seamless care and continuity for every patient. This position offers flexibility, autonomy, and the opportunity to make a profound impact daily.
Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.
Hours per week: 37.5 Monday- Friday, weekends and flexible shift patterns considered.
Main duties of the job
To contribute and support the delivery of high standard of effective nursing care, in support of and under the direction of a registered professional.
Travel across East Cheshire, providing care within each of its five care communities - you will be based in one of these areas but there will be flexibility to work across all 5
Respond to urgent, unscheduled visits for patients in need of immediate care in their own homes.
Assess patient health conditions and deliver appropriate nursing treatment and intervention
Act as a first point of contact, offering expert advice and support to patients and families
Coordinate with healthcare teams to manage patient treatment plans and continuity of care
Promote patient safety, dignity, and comfort within their home environment

Qualifications
Essential criteria
- Registered Nurse to degree / diploma level
- Current registration with NMC (Nursing and Midwifery Council)
- Evidence of recent professional development in an up to date portfolio
Desirable criteria
- Non-medical prescriber
- Teaching / Assessing qualification
Knowledge & Skills
Essential criteria
- Ability to assess clinical situations and act appropriately
- Able to assess, plan and evaluate patient care taking into account the needs and desires of the patients
- Awareness of effects of ill health and the ageing process
- Awareness of the principles and practice of SSKIN (Surface, Skin inspection, Keep moving, Incontinence, Nutrition)
- Demonstratable knowledge of assessment and therapeutic interventions in area of practice
- Ability to prioritise and organise workload effectively
- Ability to communicate in a professional way with people at all levels of understanding either face to face, on the telephone, email or written communication
- Compassionate – with exceptional interpersonal skills with the ability to communicate effectively with staff, patients, carers and relatives (as appropriate) remaining sensitive and empathetic
Experience
Essential criteria
- Recent previous experience within a comparable role / transferrable skills
- Experience of developing programmes of care of an individual or groups of patients / clients and of providing advice
Desirable criteria
- Experience of mentoring students / supervise other junior staff
- Experience of working within a multi-disciplinary team
- Previous experience delivering care in a community setting
- Experience of working in multi-agency services
- Experience of mentoring
- Knowledge and experience relevant to area of practice
- Understanding and / or experience of interagency working
