The IV at Home Team cover the footprint of Central Cheshire Integrated Care Partnership (CCICP). The team offer an MDT approach and you will work closely with consultants, pharmacy and microbiologists.

We are looking for a highly skilled Nurse who possess experience of working with patients who are being treated with Intravenous antibiotics within the community or hospital setting. You will have a broad set of clinical skills and the ability to provide specialist knowledge and skills to the IV at Home Team. This role will require you to have of managing a caseload within IV at Home Team and provide advance clinical leadership in providing safe and effective care.
You will report into the IV at Home Team Leader who will support and guide you through regular 1-1 meetings. This role will also give you experience of working with the wider MDT and our partners within the wider Integrated Care System.
We are looking for caring, understanding, enthusiastic and flexible nurse who possesses advanced clinical skills, leadership and digital skills to deliver high quality patient care.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.
We employ almost 5,000 members of staff, provide around 540 hospital beds, with a range of services including accident and emergency, maternity, outpatients, therapies, and children's health.
The Trust is also part of Central Cheshire Integrated Care Partnership (CCICP), a unique local health partnership that provides a range of community services for people across South Cheshire and Vale Royal.

JOB PURPOSE
This role requires the post holder to work within Care Communities across South Cheshire and Vale Royal. The role will work in partnership with Mid Cheshire NHS Foundation Trust to ensure the smooth and timely enrolment and treatment of patients deemed suitable and safe to receive intravenous therapy in their home. The post holder will be part of a team responsible for coordinating the care and support of all IV Therapy patients over seven days. The post holder will respond to referrals, undertake holistic assessment and coordinate the delivery of the IV therapy, this includes supporting patient/carers, communication and working closely with acute wards. The post holder will be fundamental to ensure consistent delivery of the service and maintain communication and a joined up clinical approach across community and secondary care. As the service develops, the referral sources will be broadened.
The post holder will have responsibility for the training and development on IV Therapy training courses and generally supporting Central Cheshire Integrated Care Partnership staff in providing IV therapy treatment and teaching patients how to self-administer their own IV therapy treatment. The post holder will oversee the clinical competency framework for this workload.
SERVICE DELIVERY
Ensure competence in the correct use of all equipment used in the clinical setting in accordance with instructions and standard operating procedures, reporting and responding to any faults as necessary.
To provide a resource of advice and expertise to community patients, their families, carers and other health care professionals as appropriate
Demonstrate awareness of legislation regarding IV Therapy and associated devices and deliver safe care and advice as appropriate within the community setting.
Implement and monitor systems and processes to ensure that patient needs are comprehensively assessed, appropriately planned, effectively implemented and evaluated according to changing health care needs and varying levels of complexity
Implement change and best practice in terms of patient flow, admission and discharge arrangements according to service plans and objectives, by providing clinical expertise, advice and guidance
Work collaboratively with the multidisciplinary team/agencies to ensure that patient needs are met, especially in relation to on-going care needs and discharge arrangements
Involve patients, relatives/careers in the delivery of care, providing reassurance and support to assist their understanding and co-operation
Ensure that the clinical area is safe for practice by ensuring that cleanliness is maintained, materials and equipment are ready for use, including clearing away afterwards
Recognise and respond appropriately to urgent and emergency situations and ensuring that others do so.
Collect, collate, and report information, ensuring that accurate EMIS patient records and documentation are maintained
Practice in accordance with the NMC Code in order that a safe and quality service is provided
GOVERNANCE
Promote the individual needs of the patient, by acknowledging preferences, rights and choices, respecting privacy and dignity, and by promoting anti-discriminatory practice, alerting colleagues if patient care appears to be disrespectful or discriminatory
Establish and maintain effective communication, maintaining confidentiality of information
Continually monitor standards, by identifying risks, benchmarking, audit and research, assisting their line manager in the implementation of action plans
Implement and monitor systems and processes for ensuring the effective management of risk, accurate recording and reporting, and maintenance of all records and documentation
Contribute to the content of policies, procedures and guidelines related to own clinical area, and others
Ensure compliance with Trust policies, procedures and clinical guidelines for self and others, alerting senior staff if practice appears to contravene policy, or if concerned about any aspect of patient care.
MANAGERIAL/LEADERSHIP
Have the ability to influence and negotiate and build relationships and networks across professional boundaries, both internally and externally, to ensure up to date intelligence and to ensure consistency of standards in meeting quality targets.
Participate in the development of services from the patients perspective, making suggestions for change, and by leading on improvements under the direction of the line manager.
Initiate patient and public involvement activities in the community to inform service improvements
Demonstrate effective clinical leadership by prioritising patient care, allocating, supervising and evaluating the work of the team under the direction of the line manager
Accept delegated line manager responsibilities by participating in the management of the team, recruiting staff, ensuring local induction, work-based learning and education takes place, and by providing clinical supervision, performance management and staff appraisal as required
Participate in the effective and efficient use of physical and financial resources, evaluating information to ensure staff rotas have the right numbers and skill mix of staff, equipment and supplies are available, and any concerns are reported.
Promote, monitor and maintain a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns
EDUCATION/LEARNING
Take responsibility for own learning by full participation in appraisal and clinical supervision, attending mandatory training, developing own “expert” knowledge and skills, maintaining a professional portfolio
Provide induction, work based learning and assessment of others designed to improve knowledge and clinical practice within the team, including compliance of others with regard to mandatory training. Comply with plans to improve knowledge/clinical practice within the team, including involvement in supervision and mentorship

Central Cheshire Integrated Care Partnership is looking to ensure that we provide equity of services across seven days of the week. This post may be reviewed in line with this plan and in some cases an element of weekend working may be required.
