Due to Internal promotion we have an exciting opportunity for a  full time permanent Band 6 role within our Older adults CMHT.   

We are a well-established enthusiastic and motivated team. We work with people who are experiencing Mental health issues. 

We cover Chester and the surrounding areas and are looking for qualified professional mental health Nurse who want to become part of a supportive, forward thinking Community Mental Health team.

This is a great time to be joining the team, as you will be part of developing our response to Community Mental Health Transformation. 

The post holder will work autonomously within a multi-disciplinary framework, undertaking assessments and coordinating the care of patients with both functional and organic mental illness.  

The post holder will maintain a caseload which will involve ongoing assessment: Formulation of care planning, risk assessments and Memory assessments. Completing reviews and offering interventions to help improve wellbeing/outcomes for the individual and their families, signposting to other agencies as necessary. Excellent communication, report writing, and assessment skills are essential for this post.  Applicants should have sound working knowledge of mental health disorders and the Mental Health Act.

We are a friendly and supportive team upholding the core values of CWP.  We offer a thorough induction period, which includes shadowing established team members. Supervision and training are mandatory with wider training / personal development readily available. 

Access to transport for work purposes is essential.
