Job overview

JOB PURPOSE

  • To provide specialist evidence based nursing care for sick children in the community, to reduce the length of hospital admissions / attendances where it is safe to do so.
  • To work collaboratively and in co-operation with the child and their family and other health care professionals to provide clinical care, advice and counselling support in the home environment.
  • To provide an effective liaison link between hospital and home, encouraging a unified approach prior to discharge home.

Main duties of the job

  • To assess, plan and implement programmes of care 
  • Manage a caseload of patients under the supervision of a senior nurse
  •  Work collaboratively and co-operatively with members of CAU, Home Care Team and neonatal unit to provide a seamless process of transitional care planning.
  • To participate in a weekend service for children requiring chemotherapy within agreed health and safety and home administration guidelines, following provision of training from shared care centre.
  • Identify and assess conditions that may put a child at risk of abuse and / or neglect to safeguard the welfare of children by referring to the appropriate agencies within MCHFT Safeguarding Children policy

The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.

The results in the 2020 national NHS Staff Survey showed improvements in all elements of the safety culture theme, with 92% of respondents feeling their role made a difference to our patients.  We were also recognised nationally for our workforce health and wellbeing initiatives.
