The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable.

Job overview
An exciting opportunity has arisen for committed and enthusiastic full time/ part time Registered General Nurses to work as part of the Carmarthenshire Community Nursing/Intermediate Care Service, based in the Acute Response Team.
The Acute Response Team is a multi disciplinary team that provides a wide range of interventions in a patients home.
Main duties of the job
The post-holder will be responsible for providing acute nursing interventions to patients within the community setting, thereby reducing the need for hospital admissions. The post offers the opportunity to develop your skills and experience whilst making a difference to patient care.
The post-holder will need to demonstrate that they have good communication skills and it is essential that you possess the skills to work as part of a team. The post holder must be able to work autonomously, which is needed to undertake a lone worker role in the community. You must be able to take on the challenges of a changing, modern day multi Disciplinary Intermediate Care, Community Nursing Service.
You will be expected to work from one of two bases at Eastgate, Llanelli or Glangwili General Hospital, Carmarthen.
Various full and part time hours are available
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes.
Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
