Job overview
This is an exciting opportunity to join CCS Luton Adults Specialist Palliative Care Team to support patients with life limiting illnesses and those who matter most to them, with specialist Palliative care needs, Palliative Support and advice and support in end-of-life care within the Community setting.
We are looking for a Registered Nurse associate who will work closely with The Clinical Nurse Specialists and have a key role within the team in supporting patients, families, and carers within the home environment with the aim for promoting an improved quality of life and being aware of individuals preferences and wishes while ensuring a personalised care approach.
The post holder will work with the service lead and contribute to developing service provision and ensuring the delivery of high-quality specialist palliative care within Luton Adult services and across different settings within Luton community.

Main duties of the job
Provide compassionate, safe and effective care and support to people in a range of care settings.
Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers.
Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment.
Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care.
Recognise and report any situations, behaviours or errors that could result in poor care outcomes.
Working alongside senior health professionals in planning the workload and completing tasks on Sys1/Malinko.
Participating in co coordinating patients and workload.
Being an accountable professional
Act in the best interests of the people they care for
Put people first and provide nursing care that is person centred, safe and compassionate.
Act professionally at all times
Use knowledge and experience to make evidence-based decisions and solve problems.
Recognise and work within the limits of their competence.

To be actively involved in the communication of complex information regarding clients’ needs and any other useful information to the Adult Nursing Team, and/or other appropriate medical services (such as GP) and members of the wider Adult Services team.

Qualifications and Training
Essential criteria
- Foundation Level Degree: Nursing Associate Qualification
- NMC Registered Nursing Associate
- Completion of End Point Assessment for applicants who have undertaken an apprenticeship route.
- GCSE Grade A-C English and Maths or Level 2 in Maths and English
- Evidence of continuing professional development
Desirable criteria
- Care Certificate or willingness to train.
- Related health and social care qualification (Level 3).
Experience
Essential criteria
- Experience of working in teams
- Experience of patient-centred care
- Experience of health coaching and promoting independent living
- Competent with Sys1 and competencies relevant to community nursing.
Desirable criteria
- Previous experience of working within the community setting/Working within palliative care services.
- Evidence of involvement in support / development of less experienced staff
- Willingness to learn appropriate clinical competencies.
Skills and Knowledge
Essential criteria
- Ability to communicate with members of the public and health and social care providers.
- Ability to deal with a non-routine and unpredictable workload and individual patient contact.
- Ability to manage own workload.
- Ability to work on own initiative.

- Knowledge of when to seek advice and refer to a registered health or care professional.
