We are currently looking to recruit an enthusiastic and motivated Clinical Lead (Mental Health Practitioner) to join our First Response Service.

We provide 24-hour access to mental health services for individuals of all ages experiencing a mental health crisis. If you join us, you will be part of a pioneering and award-winning team, working closely with community agencies such as the police, ambulance services, and local authorities to develop short-term contingency plans for those in crisis.  The working environment includes exposure to high levels of distress in others and at times can be high pressured. Lone working in the community is a major element of this job as is potential exposure to verbal and physical aggression.

The Clinical Lead role includes  working 1:1 with service users (both over the phone triage and face to face assessments) and working collaboratively to develop crisis and safety plans, however as a leader, you will also be responsible for supporting the wider team and will have expertise in risk assessment and safety planning.

· Co-ordinate the shift and allocate work (including referrals or assessments) to the team. 

· Conduct telephone triage and community face-to-face assessments for individuals experiencing a mental health crisis, a Clinical Lead may support with any particularly complex cases, including giving advice to other member's of the team, or taking a lead on these assessments themselves.

· Provide brief clinical risk assessments, mental health formulations, and develop short-term contingency plans for callers using 111 option 2 or referrals from other professionals 

· Liaise with other professionals and agencies, either taking referrals from them or giving advice. This may include emergency services, GP's,  local authorities, school's and care home's for example. 

· Work closely with other mental health services, including secondary care teams, primary care mental health and other acute / crisis services. 

· Implement brief solution-focused interventions to empower patients, carers, and families in problem-solving and decision-making.

· Collaboratively develop safe and appropriate care plans, utilising strong communication and assessment skills.

· Excellent understanding of the resources offered by other external agencies in order to offer people the support they require for their personal and specific needs.

Co-ordinate each shift.

Provide evidence based clinical interventions, making autonomous clinical decisions about own professional practice.

Responsibility for the development, planning and implementation of brief care and treatment interventions for individual patients in liaison with the First Response Service.

Provide assessment, planning and implementation of individual packages of care and treatment.

Ensure that individual episodes of care are delivered in a timely, effective and integrated manner.

Promote a recovery model that empowers patients, carers and relatives to be at the forefront of decision making and ownership of their packages of care and treatment.

Undertake clinical risk assessments, based on latest empirical evidence and compliant with local policies and procedures.

Act as the patients and relatives advocate.

Enable patients and carers to manage disability, loss and change.

Champion patients’ rights, including: dignity, equality, diversity, choice and respect.

When undertaking all of the above ensure good documentation is provided and entered within the appropriate systems of record sharing.

Escalate concerns.

Delegate duties and prioritise urgency of assessments.

  • Social work Qualification
  • Mental Health Nurse Qualification/ NMC Registration.
  • Evidence of continuing professional development
  • Undertaken/working towards accredited courses which enable potential Advanced Practitioner registration.
  • Teaching (clinical practice) qualification/certificate or equivalent experience

  • Advanced Nurse Prescriber Qualification / or AMPH qualification
  • Leadership qualification/training

Experience

  • Able to demonstrate commitment to high quality care and service provision
