Frailty SDEC Clinical Nurse Specialist

Band 6 £35,392 - £42,618 Per annum, pro rata if part-time

Hours: 37.5 per week as part of a 7-day working pattern, all MKUH roles will be considered for flexible working.

An exciting opportunity has arisen for the Acute Assessment and Frailty Team (AAFT) to recruit two Band 6 Frailty Clinical Nurse Specialists. This service provides a high level of holistic assessment and care for those with frailty in our Emergency Department, Frailty SDEC and within the hospital.

 

We are looking for dynamic, confident individuals with medical, elderly care, frailty or SDEC experience to provide specialist frailty assessments and treatments as part of a multidisciplinary team. You will assess, plan, implement and evaluate interventions using evidence based/best practice. You will be driven to promote a robust discharge at the earliest opportunity by liaising with multiple primary care services and virtual ward teams.

Main duties of the job

The successful applicants will be highly motivated, flexible and innovative nurses. Good communication, time management and organisational skills are essential, and you should be able to work autonomously and as part of a team. You will be responsible for supervising and training health care assistants, and a range of allied health professional students.

It is essential that you have a current NMC registration and can evidence relevant experience.

The aim of the Frailty SDEC role is to provide patients aged 65 years and over who attend the Trust as emergencies, with high quality specialised care, assessment and management. Utilising strong clinical and decision-making skills. A compassionate approach and ability to work in a fast-paced environment. 

  • Prevent unnecessary hospital admissions
  • Minimise length of stay
  • Assess and manage patients that are suitable to be transferred from ED into Frailty SDEC Beds
  • Identify risk and reduce barriers to safe discharge
  • Provide a comprehensive geriatric assessment
  • Implement the Trust dementia screening process 
  • Ensure proactive and early case management of older patients with complex needs
  • Support the delivery of multidisciplinary care and treatment for older patients with Frailty across the emergency care pathway
  • Signpost the patient to relevant community and hospital-based services including rapid access clinics

Detailed job description and main responsibilities

The successful applicants will get the opportunity to attend our new MSc Frailty module that we have developed with Northampton University.

Awareness of the Gold Standard Framework and Advanced Care Planning would be advantageous.
