Job overview

We are looking for a motivated, enthusiastic person to join the Discharge team working within Buckinghamshire Healthcare Trust. The team support the discharge of patients with varying needs.

You will work with the supervision of existing discharge co-ordinators supporting the planning, coordination and transfer of care of patients with complex discharge needs from the point of admission to safe and effective discharge.

Please note that the successful candidate will be expected to work within any of the hospital sites.

Essential requirements are set out in the attached job description. You should be able to demonstrate excellent interpersonal and communication skills, have good time management skills, and be able to prioritise and work on your own initiative as well as part of a team.

Main duties of the job

  • To support the delivery of high quality discharge and patient flow for patients with complex discharge planning needs in keeping the needs of the patient and their carer’s, acting as the patients advocate when necessary.
  • To liaise with the ward staff to identify patients with complex and multi –discharge planning needs which require intervention and work with the complex discharge co-ordinators to ensure all appropriate assessments/referrals are made and the patient’s needs are met in a timely manner in order to prevent delays.
  • Communicate and liaise with patients, relatives and carers, staff in community, social services, voluntary agencies etc to develop an optimum plan for the patient from the point of admission to discharge

EDUCATION, QUALIFICATIONS

Essential criteria
  • GCSE Grade C, or above/equivalent in English Language and Mathematics

EXPERIENCE

Essential criteria
  • Experience of working within The NHS, Private Sector or Local Authority
  • Experience of working in an environment dealing with highly confidential data
  • Evidence of good problem solving skills; ability to think laterally and creatively under pressure.

SKILLS, ABILITIES & KNOWLEDGE

Essential criteria
  • Evidence of good interpersonal skills with a wide range of people in a variety of situations and settings.
  • Clear and concise communication skills both orally and in writing.
  • Ability to work on own initiative and as part of a team
