Job overview

We are looking for a highly motivated experienced paediatric nurse to join our Paediatric Outreach Team.

The Paediatric Outreach Team cares for children and young people within their own home and primary care setting.

Currently the service provides community nursing for children and young people with acute and long term conditions, complex needs, and palliative care, including end of life care.

The children and young people can present with acute illness in addition to requiring skilled nursing intervention to manage highly complex health and developmental needs throughout childhood and at transition to adult services.

 Our services are designed to promote excellent holistic care for the children and their families.

The Paediatric Outreach Team is based within Stoke Mandeville Hospital and is part of the thriving paediatric unit, we work closely with the acute ward. Therefore you will have the opportunity to experience many specialities and get to know many of the families before visiting them in their own homes.

We are looking for nurses who have community experience or experience of managing complex medical needs of children in any setting.

The service runs from 8am to 6pm Monday- Friday and 10am-3pm Saturday, Sunday and Bank Holidays. We also provide on call service for end of life care when required

Main duties of the job

  • To work effectively with sound judgement when dealing with complex emotional situations, family dynamics and in difficult environments.
  • To work in partnership with parents/guardians and gain informed consent.
  • To create individual care plans for specific children, negotiating, agreeing, implementing and evaluating with parents and other professionals.
  • To be responsible for assessing, delivering, developing and maintaining high standards in the quality of care in the home and clinic setting. Delivering face to face support and/or telephone advice.
  • To deliver patient care including accessing central and peripheral IV lines,  IV drug administration, managing and replacing enteral feeding devices, oxygen therapy, suctioning and complex healthcare needs, wound care and End of Life Care .
  • Demonstrates the ability to manage time and resources effectively.
  • Demonstrate an ability to prioritise workload and is able to show flexibility in response to urgent and/or changing needs.
  • Through reflection and clinical support, develop strategies to improve, and where necessary change practice.

Education

Essential criteria
  • Registered Children’s Nurse
  • Teaching/Assessing Certificate or equivalent
  • Evidence of further continued professional development
  • Safeguarding Level 3 Training
Desirable criteria
  • Degree level qualification

Experience

Essential criteria
  • Extensive and recent experience in a variety of Paediatric settings
  • Experience of managing and leading a team as a Band 6 Senior Nurse
  • Experience of Quality management against current National Paediatric Standards/ Children and Young People
  • Active participation and experience of clinical governance processes

Skills

Essential criteria
  • Computer literate

  • Flexibility and ability to cope with different work environments
