The Band 7 Senior Transfer Practitioner (Senior TP) will assume responsibility for the development, delivery and monitoring of education and training within the Retrieve service, while also working as part of the clinical team to provide the specialised care required to transfer critically ill adults. There will be an expectation to continue to further develop clinical skills and support the leadership team with day-to-day operational management.

As a proficient practitioner in Retrieve, the Senior TP will demonstrate expert specialist skill and knowledge in clinical practice, act as a professional role model and clinical leader and teacher, providing effective mentorship and supervision. Identifying and pursuing educational and professional development whilst maintaining a balance between clinical practice and service improvement. The role will work closely with the Leadership team to continue to develop the service. A Senior Transfer Practitioner would be expected to lead and oversee the education and training within Retrieve. Working closely with the Peninsula Senior Transfer Practitioner for education. 

The role requires identifying educational requirement of Transfer practitioners, Transfer doctors and Duty Consultants, delivering training and education. Lead in the planning and delivery of formal and informal specialised education programmes within the Retrieve service. Oversee the induction of new Transfer Practitioners and onboarding new staff. Lead in the planning and delivery of external outreach education. Acts as a resource to other clinical areas for their specialist field as well as other national services.

The Senior Transfer Practitioner would also be expected maintain clinical competence, delivering high quality, timely critical care transfer care throughout the patient pathway working within the multi-professional consultant-led team.

Main Duties and Responsibilities

 

Professional: 

  • Be responsible for maintaining current registration with the relevant governing body (Nursing and Midwifery Council) and performing within relevant Guidelines for Practice.
  • Analyse, interpret and compare complex facts or situations and take appropriate action
  • Identifying educational requirement of Transfer practitioners
  • Work within Trust and Service guidance and standard operating procedures, as well as nationally agreed guidelines / standards.
  • Acts as an advocate for the patient and their families.
  • Leads and actively contributes to service working groups.

 

Communication:

  • Demonstrates excellent verbal communication and listening skills with patients, relatives and all the multi-professional members of staff.
  • Demonstrates the ability to manage and convey complex, sensitive information
  • Demonstrates politeness, courtesy and sensitivity in providing compassionate care for patients, visitors/relatives and colleagues.
  • Contributes towards sharing good practice within the multidisciplinary team.
  • Works cohesively with all members of the multidisciplinary team and promote effective liaison with the team in ensuring that the very best services to patients are provided at all times.
  • Communicates effectively within the multidisciplinary team, participate in motivating and supporting team members and maintain confidentiality with patients and carers.
  • Accepts responsibility for establishing and maintaining productive inter-professional working relationships.

 

Clinical Role: 

  • Delivers high quality, timely critical care transfer care throughout the patient pathway working within the multi-professional consultant-led team. 
  • Performs transfers with Duty Consultant and Transfer Doctor or Transfer Doctor with remote Duty Consultant supervision.
  • Performs Practitioner-delivered transfers with remote supervision and support from the Retrieve Duty Consultant, and typically, a doctor from the referring hospital.  Utilising advanced clinical assessment skills, triage the patient’s clinical suitability for a Practitioner-delivered transfer.
  • Demonstrates expert specialist skill and knowledge in clinical practice, is seen as a clinical leader and will supervise others
  • Promotes the needs of patients and their families.
  • Be available to Retrieve team members as a source of advice and support, in relation to specialist critical care practice and safe transfer practice.
  • Acts as a resource to other clinical areas for their specialist field as well as other national services.
  • Encourages and used evidence based practice
  • Identifies and pursues own educational and professional development, maintaining a balance between clinical practice and management skill

 

Education: 

  • Leads in the planning and delivery of formal and informal specialised education programmes within the Retrieve service.
  • Oversee the induction of new Transfer Practitioners, allocating and working with mentors
  • Involvement in the on-boarding of new medical staff
  • Leads in the planning and delivery of external outreach education.
  • Participates in education forums locally, nationally and internationally.
  • Ensures that professional knowledge and practice is constantly updated.
  • Takes personal responsibility for life-long learning and personal development through clinical supervision, appraisal and Knowledge and Skills Framework.
  • Monitor and assist in yearly competency skill framework for all staff

 

Patient Care:

  • Leads the development, improvement, management and promotion of the service ensuring the delivery of high quality, cost effective care.
  • Leads on the implementation and monitoring of adherence to relevant clinical guidelines.
  • Provides all staff with specialist advice
  • Educates and shares expertise with the multidisciplinary teams
  • Ensures optimum care to patients.
  • Supports practitioner-led processes.
  • Acts as a positive role model for nurses in the services through the demonstration of strong leadership skills.
  • Works clinically as part of the nursing team, to share experience, knowledge and expertise and role modelling to more junior staff.
  • Acts as a change agent, using skills that will motivate and reassure staff through negotiation and training, to facilitate the change process.

 

Research Responsibilities:

  • Collates and records quantitative and qualitative data that provides evidence of productivity, outcomes and quality, through audit and research.
  • Leads clinical audit and undertakes research relevant to specialty and co-operate with other affiliated work.
  • Promotes a culture of evidence based practice.
  • Works with the multidisciplinary team to develop standards of care and undertake evaluations to monitor the impact on patient car

 

Leadership and Management:

  • Planning the provision of day to day organisational tasks
  • Offers formal managerial and clinical supervision and support
  • Deputises for the Base Lead Nurse as requested
  • Assists with the recruitment and selection of staff
  • Ensures that staff can demonstrate the ability to manage workload based on flexible principles and prioritise care based on clinical need.
  • Demonstrates the ability to deal with immediate issues concerned with staff sickness and absence.
  • Maintains accurate and concise records that produce statistical data for the purpose of evaluation and development of the service.
  • Keeps accurate and up to date patient records.
  • Serves on working parties and project teams related to direct and indirect management of services as required.
  • Participates in strategic care and business planning for development of the service as required.
  • Participates in the development of evidence based policies and procedures.
  • Facilitates the development of quality initiatives such as audit, evidence based practice and risk management within a culture of continuous quality improvements.
  • Practices in accordance with the Retrieve policy and procedure
  • Participates in individual performance review annually.

Knowledge and Experiance

  • Significant recent operational experience in speciality of Intensive Care
  • Significant and recent critical care experience with the ability to demonstrate
  • Previous experience of change management and problem solving
  • Evidence of post graduate specialist training to masters level or equivalent
  • ALS provider
  • Recent relevant experience with intra-hospital and inter-hospital transfer
  • Evidence of recent teaching experience
  • Portfolio of evidence showing achievements in practice / speciality
  • Evidence of delivering teaching programmes to staff groups
  • Understanding of professional and current issues in critical care transfers

Qualifications and Training
