Community Mental Health Practitioner

We are looking for an enthusiastic and dedicated member of staff with RMN, Social Work or Occupational Therapy qualifications to join our team to join us at the beginning of their career.

You will have the opportunity to work alongside experienced practitioners of many disciplines and benefit from collaborative working and supportive colleagues.  We very much aim to promote professional development and you will have the opportunity to focus on particular interests to allow progression through the AFC bands.

You will carry a caseload of around 20 people but will be closely supervised and supported.  The caseload will consist of people with a variety of mental health problems.  You will gain knowledge and experience in assessment, planning and delivering evidence-based interventions.

Berkshire CMHT are currently undergoing a period of transformation which aims to improve service provision and experience of the service user.  It also aims to ensure our staff feel supported and that they have the skills they need to carry out their role.  This is an exciting time to join our team.

In West Berkshire, we work closely with other teams and organisations both within BHFT and external partners.  This promotes better outcomes for service users and encapsulates true team working.

Our operating hours are generally Monday to Friday 9am-5pm but in the interest of maintaining a positive work/life balance we can offer flexible working and the opportunity for remote working also.

Prif ddyletswyddau'r swydd

  • The post holder will be responsible for the formulation, execution and evaluation of a care plan for a designated number of patients, who may present with some complexity, within a multidisciplinary team.
  • The post holder will ensure that each individual plan is developed in collaboration with the patient and informed by the wider multidisciplinary team.
  • The post holder will operate within the principles of recovery and as such support each patient to develop a Wellness Recovery Action Plan to promote empowerment and self management thus reducing dependence on Secondary Mental Health Services and enabling a planned integration into local community facilities and the discharge as part of the patient’s recovery plan.

The “must haves” for this role:

1.       A professional qualification, RMN, Social Work or Occupational Therapy with a passion in mental health.
