The Community Mental Health Team in West Berkshire, based at Hillcroft House, Thatcham, is seeking an Advanced Clinical Practitioner to join our innovative and dynamic team.

We are dedicated to supporting individuals aged 18 to 75 who are experiencing severe and enduring mental illness and we support our clients to understand their experiences and return to college, university or employment.

Prif ddyletswyddau'r swydd

  • Contribute to the delivery of specialist interventions, with a particular focus on psychosocial interventions, physical health/wellbeing and supporting informed treatment decisions in severe and enduring mental illness.
  • Deliver and /or contribute to medicines optimisation using a person-values centred approach.
  • Demonstrate expert clinical knowledge through provision of highly specialist advice and support to patients, families, BHFT staff and external stakeholders. 
  • Demonstrate effective communication and liaison with other care providers in order to ensure high standards of care are maintained.
  • Deliver specialist training to BHFT staff and external stakeholders regarding severe and enduring mental illness, psychotropic prescribing and shared decision making.
  • Contribute to development and implementation of the service strategy, performance metrics, key objectives and efficient use of resources.
  • Undertake and support medicine and physical health-related audits and quality improvement projects. 
  • Lead on implementation and support the use of shared care protocols where appropriate.
  • Undertake a fluid and flexible approach to work.

Experience

  • Evidence of on-going CPD
  • Evidence of teaching in a variety of environments
  • Experience working with service users who present with challenging and complex behaviour
  • Experience in clinical audit
  • Experience in project facilitation
  • Experience of MDT mental health team working
  • Experience of providing information to mental health patients
  • Experience in supervising staff
  • Experience in training of other staff
  • Experience in carrying out medicines information queries, providing both verbal and written specialist advice

Knowledge, skills and abilities

  • Ability to demonstrate attention to detail, accuracy and numeracy/ calculation skills

  • Ability to prioritise and manage own workload
