Job overview

We are currently recruiting for a District Nurse/Community Nursing Sister/Charge Nurse to join our busy yet rewarding team. 

 

You will be an enthusiastic and experienced nurse, with a warm personality and great people skills. Willing to undertake relevant training, you will have leadership experience and be able to demonstrate innovative practice. Showing effective management skills, you will take responsibility for caseload management of housebound patients with multiple nursing need in the community. 

You will be responsible for ensuring high quality evidence-based care for patients, their families, and carers in the community. 

Our working hours/days are as follows: 

Monday to Friday (Core hours) - 08:30 – 16:30  

1 weekend every 4 weeks with days off in the week  

Late shift – 11:00-19:00  

Early shift – 07:00 – 15:00

Main duties of the job

  • You will manage a complex and diverse caseload of patients.  

  • You will provide high quality nursing care to a range of patients with varying healthcare needs. 

  • You will provide leadership , support  and role modelling to  staff, sharing learning and developing practice. 

  • Some of the healthcare you will be expected to provide includes end of life care, wound management, and much more. 

  • Full time (37.5 hours per week) and part time opportunities available 

  • Located at Upton Hospital and covering surrounding areas.

  • Qualified Registered Adult Nurse with NMC PIN 

  • Experienced Community Staff Nurse 

  • Evidence of Leadership and Management development 

  • Specialist Practitioner Qualification in District nursing highly desirable. 

  • Car Driver with UK licence 

  • Positive and motivational attitude

Essential criteria
  • Able to demonstrate recent innovative practice
  • Able to work flexibly across service areas to enhance quality of service provisio
  • Valid full UK licence with confidence to drive during and in unusual environments with use of car for work purposes.
  • A degree of agility and the ability to manoeuvre light to moderate loads.
  • Able to work bank holidays, weekends and unsocial hours to meet service needs.
  • Able to work in challenging environments.

Education/Qualifications /Training

Essential criteria
  • Registered Nurse – Adult
  • Active on the Mentor Register
  • Evidence of Leadership & Management development
  • Recognised coaching qualification or willing to undertake
Desirable criteria
  • Specialist Practitioner qualification (DN Degree, diploma or Certificate in District Nursing
  • Non-Medical Prescriber/Nurse Prescriber Qualification

Continuous Professional Development

Essential criteria
  • Evidence of continuing professional development (CPD)
  • Evidence of a current CPD portfolio

Previous Experience

Essential criteria
  • Experience in mentoring and assessing both pre and post reg. nurses in a community setting.
  • Evidence of clinical leadership within a community nursing environment including;
  • Mentoring/assessing in practice
  • Coaching
  • Risk and Incident Reporting (Root Cause Analysis) and supporting teams in learning from finding
  • Clinical Leadership course or willingness to undertake
  • Able to manage own workload and support others
  • Able to conduct courageous conversations and support staff with performance related issues
  • Experience of leadership within a team.
Desirable criteria
  • Evidence of clinical leadership within a community nursing environment including;
  • Coaching
  • Clinical Leadership course or willingness to undertake
  • At least 2 years Community Nursing experience at Band 6.
