Patient Experience Liaison Nurse (Community Outreach)

We have an exciting opportunity for fixed term roles until June 2025 to deliver our Community Wellness Outreach Programme, providing NHS health checks in the community, as a key mechanism to tackling health inequalities. The roles are part of our innovative Meet PEET (Patient Experience Engagement Team). The new programme is a joint partnership with the BOB Integrated Care Board, Reading Borough Council, and local Voluntary Sector organisations.


The majority of your time will be in the local community, delivering health checks to seldom heard but important community groups. The role involves providing health education, advice and referrals. 
We have various contract hours available (between 15 hours to 37.5 hours), to enable us to deliver the flexible programme of events, and can offer secondments as well as new posts.


Events are organised in collaboration with community partners, so session timings vary to meet their needs. Therefore, although you would have set weekly hours, actual shift patterns each week may vary. We are looking for individuals who can be flexible and work with us in supporting these events.
This is a physically active role, requiring kit transportation and set up in each location. The post holder will need to be able to drive.


We are looking for nurses passionate about addressing health inequalities and ideally with specialist knowledge to bring to our talented team of nurses.

- Deliver NHS health checks in the community in areas where health inequalities are greatest. Including providing specialist advice and referrals where required. We have a commitment to deliver 5200 health checks by June 2025.


- Be part of a multi-discipinary team, including social prescribers, voluntary organisations, housing, finance professoinals etc. You will sometimes need to respond to challenging individual cases.


- Work in collaboration with community partners, establishing strong working relations, and building trust in the community.


- Represent the Meet PEET initiative and Trust at events to promote the programme and provide updates and reports as needed. 


Key experience, skills, and knowledge:
- Registered nurse with significant experience of working with people of all ages, from diverse backgrounds, cultures and level of education.


 - Recent clinical experience at grade 6/7 including making autonomous clinical decisions. 


- Excellent personal and communication skills. Motivated, enthusiastic, flexible and resourceful. You will need to adapt to different settings, as you will be based in community venues. You will also need to be flexible in supporting different sessions, with different timings, in different locations each day as the schedule of events varies weekly. For example, some sessions start at 8.30am and some finish at 6.30pm. Occasional weekend shifts are required and support at festivals and bigger events.

These posts will be part of the Patient Experience Engagement Team which sits within the wider Patient Experience Department. This is a broad department focusing on a range of patient experience issues for the benefits of patients and staff. The team is part of the Chief Nursing Officer's Corporate Directorate.


We are an inclusive and supportive team who are all passionate about addressing health inequalities.  We support each other to deliver to our best when out in the community. 


Nurses will generally be part of a team of at least 2 nurses when out at community settings.

The main responsibilities of these posts are to:

- Deliver NHS health checks in the community in areas where health inequalities are greatest. Including providing specialist advice and referrals where required. The checks will have a particular focus on Cardiovascular Disease but also encompass wider health concerns. You will be part of a holistic team offering support to individuals. You will sometimes need to respond to challenging individual cases.

- Work across organisational and external agency boundaries to address the equality agenda in collaboration with our partners, establishing strong working relations, and building trust in the community.

- Work in collaboration with system partners for example primary care and the voluntary sector to support individuals who raise health concerns or flag in checks as needing additional support.

- Coordinate health check activities, ensuring appropriate kit is available at each session and any disposable items are ordered and restocked.

- Support and train other members of the team and volunteers attending the events.

- Represent the Meet PEET initiative and Trust at events to promote the programme widely and identify further locations and community groups to engage with. 

- Provide updates and reports as needed on the progress of the programme.

- Use data and consultation with patients to ensure services are responsive to patient needs.

- Represent and participate in patient experience forums and initiatives to improve patient experience.

- Support health promotion events, for example as part of our youth engagement programme.

- Actively promote diversity, both in the delivery of services and the management and development of staff. Identify and address discrimination and oppression.

- Identify sources of funding to enable system wide patient experience and health promotion projects within the trust and local community. 

- Maintain visibility and a high profile in the community, acting as a role model for good practice.

- Practice in accordance with professional, ethical and legal codes of Nursing and Midwifery Council, and Trust protocols and guidelines. 

- Develop own skills, taking responsibility for continuing professional development and performance, maintaining own portfolio in accordance with post registration requirements.
