Are you looking for an exciting new role in a high performing and supportive team? Perhaps you are ready for your next challengeWe have a fantastic opportunity for an experienced Nurse or Paramedic to join the Reading Urgent Community Response and Virtual Ward Team. 

The role will provide clinical assessment skills to patients in their own home and in a care home, who require an urgent response to prevent unnecessary admission to hospital. The post holder will work as part of a multi-disciplinary team to provide appropriate patient focussed care.  

The role will provide leadership and line management tom junior staff within the multidisciplinary team, including nurses, therapists and support workers, and clinical supervision and training/ education to the nurses and support workers, as required. 

You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service. 

The post holder will work 37.5hrs per week ,7 days a week, between the hours of 8am – 8pm, which will include weekends. 

Main duties of the job

  • To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service in collaboration with the Community Geriatrician and GP. 

  • As an experienced nurse practitioner, you will take responsibility for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines. 

  • The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner 

  • To provide leadership and line management to the Band 6 nurses and the Band 3 & healthcare/multi-therapy assistants. 

  • Ensure accurate, contemporaneous documentation and care plans are maintained accordance with the NMC, information governance and local / national guidance. 

  • Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery. 

  • Work flexibly in response to service need across our multiple sites

  • Registered General Nurse or Paramedic or Physician Associate  

  • Advanced History taking and physical assessment or working towards it 

  • Non-medical prescriber or working towards it

  • Substantial post registration experience in hospital and/or community pharmacy

Education/Qualifications/Training

Essential criteria
  • Registered General Nurse or Paramedic or Physician Associate
  • Post graduate diploma or equivalent experience
  • Advanced History taking and physical assessment or working towards it
  • Non-medical prescriber or working towards it
  • Qualification in Teaching and Assessing

Previous Experienc

  • 3 years post registration experience
  • Demonstrate a high level of clinical skills.
  • Demonstrate an in-depth knowledge and experience of clinical examination, history taking, pharmacology
