Senior Mental Health Practitioner - Community MH Team (Elderly)

Are you an experienced, registered Mental Health Nurse, Occupational Therapist, or Social Worker seeking a rewarding community-based role with a Monday-Friday, 9-5pm working pattern? Come and join us in our CQC rated ‘Outstanding’ Trust! We’re on the lookout for someone with specialist expertise in the care of older people to join the Community Mental Health Team (elderly) where we provide mental health care to older people with mental health conditions alongside co-existing physical health challenges. 

In this role your experience in Community Mental Health Team settings will be invaluable as you deliver personalised care and treatment, adhering to established pathways including the Dementia Pathway and NICE guidelines. 

 You will need to have excellent communication and people skills and ideally, you’ll have a good understanding of Mental Health Framework and legislation, the Mental Health Act & the Mental Capacity Act as well as Clinical Risk policies and procedures.

While most team meetings are currently held virtually, occasional in-person gatherings for meetings, training, or other service-related matters may be required in the future.

Based at Nicholson House in Maidenhead, your days will be spent conducting client visits in the local community, making a car essential for your appointments.

  • Undertaking initial, comprehensive assessments in a range of settings including patient’s own home, taking in to account the person’s physical, psychological, social, and spiritual needs.  
  • Developing an agreed care plan with the service user and where appropriate include the view of the families and carers. 
  • To monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team by reporting in review or at CPA meetings.  
  • Managing your own caseload, working collaboratively with other disciplines from both statutory and voluntary sectors. 
  • Responding to acute psychiatric problems and able to take new referrals.  
  • To have knowledge of a variety of therapeutic interventions e.g. short term problem solving, crisis intervention and basic CBT techniques. 
  • Participating actively in Service Development contributing to projects/plans aimed at improving and maintaining and quality of care.
