The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable.
Job overview
The position holder will be responsible for caring for children with complex health care needs in the community setting either at home or school. This is an extremely rewarding area of practice caring for children with life limiting/ life threatening conditions. The post holder will provide hands on nursing care to these children following training and assessment of competence in various nursing skills. Full or Part time hours available.
Main duties of the job
The post holder will be required to be caring, compassionate, dedicated and flexible in approach. An ability to work well on own initiative and within a small team. The post holder will be required to work shifts including unsociable hours, weekends and nights.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Detailed job description and main responsibilities

1. Work flexibly as part of a Multidisciplinary Team
2. Undertake direct patient care as delegated by the professional team and within the individual child’s plan of care as appropriate to competency levels.
3. To undertake duties appropriate to competency levels e.g. suctioning, gastrostomy feeding, tissue viability care, personal care and other designated nursing tasks.
4. Encourage the child’s participation with personal and daily domestic activities,
5. Supervise the child in activities such as mobilising, bathing and ensure the correct and safe use of patient equipment e.g., hoist, showering equipment and frames.

Qualifications and/or Knowledge
Essential criteria
- QCF Level 3 or equivalent qualifications/experience
- NVQ level 3 in Health & Social care related subjects or equivalent qualifications/experience
- Good knowledge of anatomy & physiology
Desirable criteria
- All Wales HCSW induction framework
Experience
Essential criteria
- Experience of working as a HCSW without direct supervision in a clinical setting and being managed by a qualified practitioner.
- Good understanding of risk assessment and management
Skills
Essential criteria
- Evidence of Being able to provide wound care using aseptic technique ANNT
- Phlebotomy
- Basic observations
- The ability to communicate clearly and accurately. The ability to be adaptable to new and changing circumstances.
- The ability to work alone under supervision and to understand when to seek clinical advice and support from the qualified Nurses
Desirable criteria
- Vital signs, recording (BP pulse, temp, respirations, blood sugar)
- Venepuncture
- Routine ECG Recording
- Urine testing
- Any other specific clinical skills at this level of practice pertaining to role as per service need Knowledge and understanding of clinic activities
