Job overview

This is a post in primary care within the community mental health team  in Conwy

We provide assessments from numerous agencies who refer through our SPOAA daily meetings, and offer brief interventions within groups and occasionally on a one to one basis.

Also a good knowledge of third sector agencies is useful as we work hard to become more familiar and signpost to these very helpful agencies, moving on in improving our contact with them and continually improving our working relationships.

Main duties of the job

An exciting opportunity has arisen in Primary Mental Health Support services for an experienced, motivated and forward thinking practitioner to join a busy supportive team based in the Conwy area. 
A position have arisen in Conwy Community Mental Health team.

The successful candidate whilst working closely with a multi – disciplinary team, will receive referrals directly from GP’s and provide short term interventions for individuals with mild to moderate illness and will work closely with colleagues from secondary care and specialist services as well as third sector organizations and statutory agencies. 

The successful candidates will manage a caseload of patients with whom they will work alongside to formulate risk management plans and care planning based on robust assessment of need. We are looking for a good team player who has excellent communication skills and the ability to prioritize and manage their time effectively.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
