Our INTs are large teams made up of nurses, therapists, paramedics and support workers who work in an integrated way to support service users on their healthcare journey.
We are able to offer a range of flexible working options and are offering part time and full time contracts. If your application is successful this is something we can discuss at interview.
Main duties of the job
Complete holistic assessments of service users.
Develop and deliver person centred care plans.
Line manage junior staff and contribute to their development through supervision, 1:1s and training.
Undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and a deliver a competent clinical service.
Communicate with the senior leadership team in the locality to ensure smooth delivery of service.
Identify risks and act accordingly to minimise risk to self, service users and colleagues in line with Organisational policy and procedures.
Coordinate shifts and delegate/allocate care to the team in accordance with policy.
Act as a point of escalation for staff to report to on discovering any issues or deteriorations.
Undertake further training as required in order to develop extended skills and enhance patient care.

Qualifications
Essential criteria
- BSc or equivalent experience in Adult Nursing or an Allied Health Professional role
- Current NMC registration
- Evidence of continuing professional development and commitment to lifelong learning
Skills, knowledge and experience
Essential criteria
- Effective communication skills written, verbal and non-verbal with the ability to adapt communication for individuals with communication difficulties.
- Able to effectively lead a team with senior support when required
- Able to make clinically reasoned judgements when working with a variety of conditions
- Experience of supervising staff and students of own profession
- Effective time management and caseload management
Desirable criteria
- Recent relevant experience within the community setting including management of the frail complex patient
- Willingness to undertake additional study and qualifications relevant to the role
- Experience of the application of health, safety and risk management policies
Applicant requirements
You must have appropriate UK professional registration.
