Please be advised that there is a temporary top up for Bands 1,2 and 3 to reflect the incorporation of the top up to the  living wage of £12 per hour - £23,465 per annum. This temporary top up will be in place until  the annual pay uplift for 2024/25 is confirmed

 

Job overview

We believe in developmental and training opportunities for our staff and therefore are currently seeking to recruit highly motivated, passionate Band 5 Community Nurses who are keen to develop and work towards one of the following roles;

• Band 6 District Nurse by undertaking the District Nurse Qualification.

• Chronic Condition Case Manager by undertaking disease specific training to provide evidence based care to those individuals with highly complex needs.

• Nurse Practitioners by undertaking a modular approach to attain the clinical skills required.

Main duties of the job

The successful applicants will need to have excellent communication skills with an ability to provide clinically effective care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across the Counties of Anglesey and Gwynedd working in an integrated way with partner organisations to deliver high quality evidence based care in order to maintain patients in their own homes and communities, averting hospital admissions, empowering the patient towards self management and implementing seamless patient centred care and treatment.

You must be able to travel between patients homes both within towns and rural areas in a timely manner.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
