At Ideal Complex Care Services- Complex Care Team we are looking for a Registered Nurse who can offer a high standard of clinical skills, from Branch and in home-based nursing with clients, both adults and children, with complex care needs. Your main role will be to oversee the care of our clients and service users both clinically and operationally.

Duties

· To show by personal example the highest standard of patient care and participate in active nursing care including the full use of appropriate nursing equipment.

· Initial assessment of potential clients care needs, through to the formulation of the RAD/care plan.

· Inclusive regular communication with the wider MDT and to liaise effectively with other professionals and significant others involved in the patients care, ensuring an integrated pattern of service delivery.

· Reviewing of documentation and taking appropriate actions and monitoring the delivery of care

· Teaching and assessing of skills for care staff – managing staff competencies and delegations

· To write the client MARs each month and liaise with relevant GPs/Consultants to ensure you have the most up to date medical information

· To stay up to date on any changes in the patient’s condition and for this information to be entered in the patients care plan. To report any changes to the care team.

· To assist in the review of patients Care Plans.

· To assist in the supervision and training of team.

· To conduct a professional manner between yourself and patients, i.e. no exchanging of telephone numbers, no outside contact, this could lead to a dismissal from the agency.

· To conduct spot checks during the care being provided by our field staff in the clients’ home. These spot checks will be conducted outside of the normal office hours

The postholder will have regular contact with vulnerable people and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
