Are you a nurse looking for a supportive team and the opportunity to develop new skills? This is an exciting time to join the Twilight Community Nursing service in West Herts.  We are developing new community pathways which will support the delivery of more care in people’s homes, and to enable us to do this we have a range of new learning and development opportunities available through the CLCH Academy. 

We are looking for a part-time community nurse deputy team lead with a genuine interest in continuing to develop skills in this field.  We have a good track record of supporting return to practice nurses, and nurses transitioning into the community from other settings including acute care.  You must be able to work flexibly according to service needs and have excellent organisational, time management and communication skills. 

We are looking for staff to work from 4.30pm to 8.30pm

You will be based at one of x4 sites within the Herts Valley area: Watford (BRE), Hertsmere (Potters Bar Community Hospital) St Albans and Harpenden (Harpenden Memorial Hospital) and Hemel Hempstead (The Forum, Marlowes)

Main duties of the job

The Community Nurse Deputy Team Lead (DTL) will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The Community Nurse will work in patient/carers own homes as well as visiting patients in residential homes.

The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The Community Nurse DTL will promote optimum independence and individualised client centred care to a delegated caseload.

The post holder will also support the Community Nursing Team to deliver against key performance indicators and maintain quality of the service. The service covers five localities across Herts Valleys working 4pm-8pm across 7 days. The Community nurse DTL would be expected to work a pattern of 4.30pm-8.30pm to enable continuity of care handover to the Overnight services.

  • NMC registered NHS qualification
  • Willingness to undertake training as appropriate

  • Evidence of other post – registration education and training
