Come and join our CQC rated ‘Outstanding’ Trust as a Community Healthcare Assistant in Bracknell. As a Community Healthcare Assistant, you’ll help deliver care to people in their own homes and get to work with inspiring, skilled teams whilst you do it. We offer a supportive environment with including completion of the Care Certificate, a recognised qualification, and the stepping forward programme, an enhanced training programme for Healthcare Assistants. Our team provides multiple pathways to nursing including pathways for internationally qualified nurses.   

The role of Healthcare Assistant is fundamental to the success of Community Nursing TeamsThe post holder will be able to demonstrate the competencies required to perform at this level and role model the Trust’s core values as well as exhibit the behaviours required as part of those values. 

Prif ddyletswyddau'r swydd

  • You will visit a number of patients on a daily basis in their own homes. 

  • You will provide high quality nursing care to a range of patients with varying healthcare needs. 

  • Implement, monitor, and maintain high standards of care delivery at all times. 

  • Communicate effectively within your team, the patients and their carers, and the wider MDT. 

  • Deliver high service standards, sharing best practice, and actively improving the way we work. 

  • Maintains accurate and timely patient records in line with organisational requirements. 

  • Undertake specific clinical tasks, when trained, which include: 

  • Completing a set of observations including, blood pressure, temperature, oxygen saturation, respiratory rate and heart rate 

  • Completing wound care, using a wide range of dressings including bandaging, compression hosiery and topical cream application 

  • Pressure area care as appropriate, which includes ordering equipment. 

  •  Venepuncture 

  • Catheter care – Administering catheter flushes and changing catheter bags.
