Intensive Support Team (IST)
Band 6 Community Learning Disability Nurse
The Intensive Support Team is a service designed to support individuals with a learning disability who are struggling with mental health crisis and/or behavioural distress which is placing them at risk of admission to hospital. The team is expected to meet the holistic needs of service users and carers, avoiding hospital admission where possible.
We are looking for a nurse with the passion and clinical skills to join our innovative Intensive Support Team. The Consultant Nurse leads the team of dedicated Practitioners to supporting a team caseload of people needing intensive support. There are three locality arms of the team (Spokes) who work in partnership with local service users and key agencies providing vital support in helping people remain safe in community.
An opportunity has opened for a community nurse with relevant skills and experience to join the team. The service is very much committed to providing a working environment conducive to staff development and wellbeing and the post holder will be part of a team approach to offering interventions to people who are at risk of breakdown in care and admission to hospital.

Main duties of the job
Being a community nurse in the IST gives you the opportunity to:
- Help the most vulnerable people at risk of care breakdown or hospital admission.
- Provide a variety of direct patient care interventions.
- Be part of an innovative team striving to ensure people receive good quality care.
- Develop your clinical skills around positive behavioral support, risk management, managing mental health conditions, and building independent living skills.

The Post holder is expected to work 4 days per week, over two long shifts (8-8) and two standard shifts, which include rotating weekend cover to meet the needs of team and Trust network.
A means of transport is essential, and applicants must be willing to undergo an enhanced CRB check.

- RNLD with a minimum of 3 years’ post-registration experience
Knowledge

- Understanding of contemporary Learning Disability agenda
- Knowledge and skills in the assessment / management of risk
- Understanding of PBS principles and how to apply these to promotes pro-active and preventive strategies, includes teaching new skills and implementing strategies to avert crisis and keep people safe.

- Experience of analyse and resolve problems.
