Physical health in Mental Health Nurse

As an integrated Mental Health, Learning Disability and Community Trust, Lancashire & South Cumbria NHS Foundation Trust provides a range of services including:
- Primary and secondary mental health care for children and adults including specialist inpatient child and adolescent mental health provision, perinatal mental health, forensic services including low and medium secure care.
- Specialist community support for children and adults with learning disabilities and autism, including intensive support.
- Community physical health and well-being services for children and adults.

Our Forensic Community Service is expanding, and we are excited to introduce a brand-new role that will support our commitment to holistic care. We are seeking a Physical Health in Mental Health Nurse to join our multidisciplinary team (MDT).
This role will be pivotal in bridging the gap between physical and mental health care, addressing health inequalities for our service user population and ensuring our service-users receive integrated, whole-person care. The successful applicant will be dedicated, skilled, kind and approachable with an excellent team working approach.

Prif ddyletswyddau'r swydd
- Comprehensive Assessments: Conduct thorough physical health assessments for individuals with mental health conditions within the forensic community.
- Physical Health Care Plans: Develop and implement person-centred care plans that address both physical and mental health needs, in collaboration with psychiatrists, pharmacists, mental health practitioners, and other healthcare professionals.
- Health Education: Provide education and support to service-users on maintaining physical health, managing chronic conditions, and adopting healthy lifestyle practices.
- Monitoring and Follow-Up: Regularly monitor service-users' physical health conditions, manage medication side effects, and coordinate with primary care providers for seamless care.
- Advocacy and Support: Advocate for the physical health needs of service-users within the forensic mental health caseload, ensuring they receive the necessary medical attention and support to engage.
- Collaboration: Work closely with the forensic community team to ensure a cohesive approach to service-user care, participate in care reviews, present physical health reports and contribute to team decision-making and processes.
- Training and Development: Educate and train staff and partner agencies on common physical health conditions for the caseload, treatment and management and promoting a culture of holistic care.

- Degree qualification in relevant subject or demonstrable evidence or equivalent experience relevant to field of practice.
Knowledge

- In-depth understanding of the physical health needs of individuals with severe mental health issues.
- Knowledge of relevant legislation and best practices in forensic mental health care.
- Awareness of health promotion, infection control and disease prevention strategies.
Experience

- Previous post qualifying experience, including 12 months working within a mental health setting.
- Significant experience in physical health care.
- Experience in conducting health assessments and developing care plans.
