Dual Diagnosis Outreach Nurse

Job overview

Are you a fantastic advocate?  Do you want to help improve the health outcomes and reduce health inequalities for people experiencing dual diagnosis and homelessness?  

We have an exciting new role working across North Yorkshire Horizons  and the Homeless team of Harrogate.  The DD nurse will sit with the Multiple Disadvantage Outreach team, a specialist team of clinical /non-clinical staff who engage and support some of the most vulnerable  people. Providing responsive, flexible care in a least restricting manner, taking care and interventions to where the individual feels  comfortable to access help.

Whilst working as a dual diagnosis nurse, you will work as part of a multi-disciplinary team, to break down barriers experienced by the individual, leading a trauma informed approach, coordinating effective, accessible support and interventions,  improving health and social stability.

The role will hold a small caseload of individuals who are deemed  homeless within Harrogate District alongside having a dual diagnosis. They will be expected to be highly motivated and have good ability to prioritise their caseload.  They will be expected to work  in the community across organisations, soup kitchens, street outreach as required.

Main duties of the job

Assessing individuals experiencing homelessness, substance misuse and mental health problems.

To co-ordinate the MDT team delivering care that is accessible, cohesive to the individual.

To hold a small caseload.

To provide harm reduction interventions.

To build therapeutic relationships with individuals on the caseload.
