This is an exciting opportunity for a qualified District Nurse with experience as a band 6 caseload holder, to undertake a leading role in the provision of District Nursing services for patients within an integrated health and social care team, working in a designated network area.

the successful post holder will provide proactive leadership, management, support and advice to a team of District Nurses in a designated area within Bridgend.

The post holder will join a forward thinking team working closely with other health and social care professionals to develop established integrated services and patient centred care within the community setting. The post holder will contribute towards clinical governance and the continuous improvement in quality service delivery.

The ability to speak Welsh is desirable for this post.
